Surrogacy

Also called contractual parenting, surrogacy is a reproduction method whereby a woman agrees to carry a child to term for a contracted party. The most common type of arrangement is traditional surrogacy in which the surrogate mother is impregnated with the sperm of the intended father. The child may be conceived by sexual intercourse, artificial insemination using fresh or frozen sperm or impregnated via intrauterine insemination, or intracervical insemination which is done at a fertility clinic. Another arrangement is called gestational surrogacyor the host method in which the fertilized egg of the intended mother is implanted in the womb of the surrogate mother.

Controversial issues tend to surround surrogacy. Based on research, surrogacy is the least acceptable of all artificial reproductive technologies. However, surrogacy is preferred by some couples because it it gives them an opportunity to have a child that is genetically related to them. Moreover, it can can be a quicker process than adoption.

Coping with infertility

For most people, being unable to conceive successfully results in frustration, anger, feelings of loneliness, loss and grief. There is no quick and easy formula for getting rid of the emotional burden caused by one's inability to conceiveget pregnant or to carry a baby to term. Below are some of the ways that can help couples to positively cope with infertility

Talk to others: Regrettably, our society often fails to recognize the burden caused by infertility. Some people tend to hide how they feel, later on, this can result in feelings of shame and isolation. Talking to other people who are going through the same thing can help you realize that that you are not alone. You can get support from professionals, such as counselors. If possible, look for a counselor specializes with infertility issues.

Let go of negative emotions: Everyone needs to exert efforts to be emotionally healthy. Like any disappointing news, accepting the fact that you won't have a child is something that will not happen overnight; it's a fairly long and complicated process for may people. One way to help you close in on acceptance is by trying your best to let go of unpleasant feelings such as bitterness, sadness, or regret.

Keep up with your other interests: Infertility treatment can be a long and tedious process and many couples find the agony of waiting quite unbearable. Look at the process as bus ride, the more you ask the question 'are we there yet?' the more you would get bored. Hence, you should consider diverting to attention to other things such as activities or hobbies that could relieve stress and bring you pleasure. <font> </font>If If you have outgrown old activities, continue looking for new diversions. Consider learning new things or think of those activities that you have been longing to try.

Reassess your own reasons for wanting children: When you’re preparing yourself to accept the possibility that you may not have children, there's a need for you to look at your reasons for wanting them. Regrettably, many people want to have children because somehow it is what our society dictates. Some may decide to have kids just because everyone thinks that it is right. Ask yourself again why you want to have children, and whether you are really prepared for the challenges of parenthood.

Robert Greene, M.D. provides some specific principles on fertility and exercise:

Principle One: Start a balanced exercise program which should incorporate aerobic activity, strength-building exercises, and flexibility training. If you are currently inactive, it typically takes two to three months to achieve a healthy level of fitness.

  • Aerobic Activity: The ideal goal is to perform aerobic activity for 40 to 60 minutes per day. Those that are new to aerobic exercise, should start with 20 minutes a day and increase to 30, or it can broken up into 2 or 3 20-minute sessions.
  • Strength Training is important for building muscles and strengthening bones. To maintain your muscle strength, it should be done twice a week, allowing two days of rest in between workouts. To build muscle, it should be done three times a week. Types of strength-training include:
    • Free weights
    • Resistance Bands
    • Gym Machines
    • Fitness Balls
    • Isometric (Pushing against a stationery object)
    • Power Yoga
    • Push-ups, sit-ups
    • Climbing

Flexibility and balance improves the range of motion of joints and muscles to help your coordination and agility. Activities that promote balance and flexibility include:

  • Surfing
  • Skateboarding
  • Pilates

Principle Two: Activate your Lifestyle by staying active even in your “downtime”. A study of a group of students that were given 1000 extra calories a day found that the students that were very active did not gain weight. In this case, active is defined by informal everyday activities such as standing, fidgeting, or just walking around. Some suggestions for burning these extra calories without really trying are:

  • Avoid sitting too much. Even when you are talking on the phone, you can get a headset and walk around your office.
  • Park a little further away when you go to a store or out and about so that you can give yourself a little extra activity.
  • Fidget when you are sitting at your desk or when you are sitting in your car.
  • Take the stairs instead of the elevator to burn 5 to 10 calories per flight of stairs.
  • Good posture such as sitting up straight burns more calories as it requires your body to engage core muscles.
  • Take an after-dinner stroll so that your calories will be stored in the muscles rather than your fat cells.

Principle Three: Cut back on excessive exercising. Studies have shown a relationship with vigorous exercise and fertility issues. Some warning signs of excessive exercise are sleep disturbances, prolonged soreness, chronic muscle soreness, missed menstrual cycles or an elevated resting heart rate. Even if you are not exhibiting these symptoms but suspect that you are overdoing it, here are some ways to cut back:

  • Limit vigorous exercise such as running or aerobics to three times a week.
  • If you are missing the workout frequency, try strength training or power yoga
  • Give your muscles a break so that they can get a chance to rest and regenerate.

Principle Four: Create a longer lasting change so that exercise becomes a habit. Some suggestions for creating this type of change are:

  • Try a new activity. Researchers have found that the brain’s pleasure center lights up when you engage in a new activity
  • Listen to music during exercise to pick up your pace (pick songs with a fast beat) and help the time to pass more quickly
  • Seek out a fitness trainer or fitness buddy to help you stay accountable and motivated. A fitness pro can also help start you off with an exercise routine that can help you achieve your fitness goals.

Exercise is a necessity in increasing fertility with men and women. First, exercise helps to reduce insulin resistance by allowing the muscles to more efficient in taking in glucose, helping to keep blood sugar levels stable. Exercise also lowers stress hormones which can get in the way of fertility. It improves blood flow to the pelvic area so that the uterus and ovaries have a strong oxygen supply, improves immune function by lowering inflammation (which can interfere with implantation of the fertilized egg), helps you have more energy and normalizes circadian rhythm so that you sleep more soundly. Numerous scientific studies have been conducted that demonstrate the effectiveness of acupuncture therapy in the treatment of infertility, including in women with anovulatory cycles, and those receiving IVF. The results of these clinical studies conclude that acupuncture increases the effectiveness of IVF, as well as being effective as a stand alone treatment for male and female infertility.

  • Women who received Acupuncture with IVF had 66% more successful fertilizations than the control group who did not receive acupuncture in a sample of 160 women, in a peer-reviewed study conducted at Columbia University in 2002.
  • Pregnancy rates in 60 cases of uncomplicated lacteal phase defects was 56% after 3 months in Nan Jing Treatment hospital in a peer-reviewed study conducted in 2008.
  • In a study of 28 men receiving acupuncture for infertility, the control group was unchanged, while the treatment group had significant changes in sperm counts, normality, motility and apoptosis.
  • In a study of 10 women conducted in China, acupuncture replaced HMG and HCG to induce ovulation in 10 of 11 months without causing any side effects.
  • In a study of 53 women conducted in China, 41% women with significant lacteal phase infertility became pregnant with the use of Chinese Herbal Medicine alone. The remaining 68 % required Acupuncture.

You may take the capsule two capsules twice daily with a full glass of water at mealtimes. To ensure the correct dose for liquid forms, measure it with a dropper or a dose-measuring spoon or cup.

Tea: You can make tea with fresh stinging nettle leaves, dried leaves or dried root. Pour 2/3 cup of boiling water over 3 - 4 tsp of dried leaves or dried root and steep for 3 - 5 minutes. You may drink three to four cups of nettle tea per day. Always drink additional water along with the tea.

The dosage for nettle root extract of 1:1 strength is 1.5 mL, three to four times daily.

Tincture forms (1:5) 1-4 ml three to four times daily.

Take nettle by mouth as directed. Check directions and warnings on the product package. Consult your doctor or pharmacist if you are uncertain about any of the information on the label.

Do not use different forms of nettle at the same time, unless specifically directed by your doctor. Taking different formulations together increases the risk of an overdose.

The specific methods in ART are:

  • In Vitro Fertilization (IVF) is the most successful and commonly used form of ART. It is often used when a woman's fallopian tubes are blocked or when a man produces too few sperm. First, the woman is treated with a drug that causes the ovaries to produce multiple eggs. Once mature, the eggs are removed from the woman. They are put in a dish in the lab along with the man's sperm for fertilization. After 3 to 5 days, healthy embryos are implanted in the woman's uterus.
  • Zygote Intrafallopian Transfer (ZIFT) or Tubal Embryo Transfer is similar to IVF except the very young embryo is transferred to the fallopian tube instead of the uterus.
  • Gamete Intrafallopian Transfer (GIFT) involves transferring eggs and sperm into the woman's fallopian tube. The fertilization occurs in the woman's body rather than in a laboratory. This method is not commonly used.
  • Intracytoplasmic Sperm Injection (ICSI) is often used for couples in which there are problems with the sperm. This method involves injecting a single sperm into a mature egg. Then the embryo is transferred to the uterus or fallopian tube.

Clominphene Citrate is prescribed to induce ovulation for women who have ovulatory disorders that might be the reason it is hard to get pregnant. It influences how the hormones for fertility and ovulation work in the body. Clomiphene Citrate stimulates your pituitary gland to secrete higher levels of FSH and LH-the hormones that cause the ovary to produce and release a mature egg.

The structure of Clomiphene is similar to estrogen, allowing it to bind to the estrogen receptor sites. There are estrogen receptor sites in various tissues of the body. Once these sites are filled with Clomiphene, they can no longer bind with the estrogen circulating in the blood. As a result, your body perceives estrogen level is too low. To compensate for the 'low levels of estrogen', the pituitary gland produces more FSH. The raised level of FSH will stimulate maturation of an egg within an ovarian follicle and eventually leads to the release of one or more mature eggs.

Bromocriptine is available in tablets. Initial dose recommendation is 2.5 milligrams per day. The tablet is taken by mouth. The dosage can be increased by your doctor if lower doses do not seem to produce desired effects.

Typically, it takes about six to eight weeks for Bromocriptine to begin to work, it is therefore important that you do not stop taking this medication too early. Bromocriptine should be taken until pregnancy occursyou get pregnant. Bromocriptine can be taken vaginally if its side effects become intolerable.

Over decades, several studies were done to determine the efficacy of Bromocriptine in prolactin-related disorders. Many of these studies prove that Bromocriptine treatment helps in restoring menstruation and ovulation. Use of bromocriptine has also been linked to increased pregnancy rate.

In the mid-70's a small study conducted by Finnish scientists showed that 2-5 mg twice daily dose of Bromocriptine was effective for patients with amennorhea (absence of menstrual bleeding), secondary to abnormal prolactin levels. The investigators reported that Bromocripine therapy resulted in restoration of menstrual cycle and ovulation. The most common reported side-effects were nausea and vertigo which lessened after adjusting the dose.

Bromocriptine has also been found to prevent miscarriage in patients with abnormal prolactin levels. This study was conducted by Dr. Fumiki Hirahara and his colleagues from Yokohama City University School of Medicine. From 352 women with recurrent spontaneous abortion, the investigators identified 64 patients with a prolactin disorder that was not associated with any other abnormalities, including ovarian or endocrinologic disturbances, polycystic ovaries, hypersecretion of LH, galactorrhea, or thyroid disorders. The investigators noted that the percentage of successful pregnancies was higher in the patients who were given Bromocriptine than in the group that were not on Bromocriptine therapy (85.7% versus 52.4%, P < .05). The prolactin levels during early pregnancy were significantly higher in patients who had miscarriage than in patients whose pregnancies were successful. The investigators concluded that sufficient circulating levels of prolactin may play an important role in maintaining early pregnancy, especially in patients who had recurrent miscarriages caused by hyperprolactinemia.

Bromocriptine may enhance fertility in women with prolactin disorders. Bromocriptine works by mimicking the action of certain neurotransmitters. By doing so, excess production of prolactin is prevented. Bromocriptine acts like neurotransmitters, causing your pituitary gland to stop prolactin production. When prolactin is within normal range, secretion of hormones for ovulation will resume, making it easier to get pregnant.

Trade Name: Parlodel

Bromocriptine (Parlodel) is a fertility medication that belongs to a class of drugs called dopamine receptor agonists. It has some of the same effects as dopamine, a neurotransmitter which occurs naturally in your body. Bromocriptine works by suppressing the the production of prolactin, a hormone produced by your pituitary gland.

High levels of prolactin disrupts the production of follicle stimulating hormone (FSH) and luteinizing hormone (LH), the hormones that play an important part in ovulation.

One of these studies was done by researchers from the University of Miami School of Medicine. The investigators wanted to find out the effectiveness of Human Menopausal Gonadotropin (hMG) with intrauterine insemination (IUI) for various causes of infertility and to identify the factors that could contribute the successful of the treatment. The participants has infertility problems related to male factor, oligoovulation, tubal or pelvic factor, endometriosis and idiopathic infertility.

Using a retrospective chart analysis, investigators found out that Human Menopausal Gonadotropin (hMG) with intrauterine insemination is effective for some causes of infertility. Data also showed cases of multiple births. Investigators also found out that Human Menopausal Gonadotropin with intrauterine examination did not yield successful results for some patients including those who did not achieve pregnancy get pregnant in three cycles of treatment, females more than 40 years of age, dosage of more than 300 IU daily, and those who had more than 8 mature follicles when hCG was given.
